Three different jobs, not three competing brands
If I had to compress twenty years of injectable practice into one sentence, it would be this: fillers change shape, bio-stimulators change tissue, and skin boosters change surface quality. Patients arrive asking which is best, but that question has no answer until we agree what is actually bothering them.
Dermal fillers: immediate, measurable, reversible
Hyaluronic acid fillers restore volume where anatomy has lost it. The result is visible the same day, lasts nine to eighteen months depending on product and area, and, crucially, is reversible: hyaluronidase dissolves it within minutes if that is ever required. That reversibility is a genuine safety feature and one reason I favour established hyaluronic acid products over permanent or semi-permanent alternatives.
What fillers cannot do is improve the quality of the skin covering that volume. A well-supported cheek with poor skin quality still reads as tired.
Bio-stimulators: slower, deeper, yours
Collagen bio-stimulators such as Sculptra and Radiesse work by prompting your own fibroblasts to produce collagen. Nothing dramatic happens on the day of treatment, and that is the point. Over the following weeks and months, tissue quality changes rather than volume being added, which is why the result reads as looking well rather than looking treated.
The variable most patients never hear discussed is dilution. Radiesse injected neat behaves as structural scaffolding; hyperdiluted, the same product becomes a regenerative skin treatment. The syringe is identical. The strategy is not.
Skin boosters: surface quality, nothing more
Skin boosters are injectable hydration and bio-remodelling agents. They do not lift, they do not project, and they are not intended to. What they change is how skin behaves: hydration, fine surface texture, light reflection. For a patient whose only complaint is dullness or crepiness, a booster is the appropriate answer and a filler would be an overreach.
Where the real skill sits
Almost every good plan I build is a layered one. Bio-stimulation to rebuild the foundation, filler to refine specific structural transitions, boosters to finish the surface. I explain it as construction: framework first, interior detailing afterwards. Regeneration below, refinement above.
And sometimes, after assessment, the honest recommendation is none of the three. If the concern is better addressed by sleep, sun habits or time, I will say so. There is no commission structure here, so there is nothing to sell you.

Hyaluronic acid fillers can be dissolved with hyaluronidase. Biostimulators generally cannot, which is precisely why they are placed conservatively, in stages, with review between sessions. Reversibility is a genuine consideration when choosing between the two.
Far less than most marketing suggests. The same product produces entirely different results depending on plane, volume and dilution strategy. Technique and assessment decide the outcome; the name on the box is close to the least important variable.
